New Change of Information Protocol

New protocol is in place for reporting a change in personal or professional information.

RPLSs and SITs will use the new Change of Information Form, linked to the drop down menu under SIT, RPLS, or LSLS, to change their name or contact information. This form can also be used to report the retiring of a license. To change a firm association, the Change of Information FIRMS Form must be used.

Firms or Branch Offices that would like to change their name, contact information, noted management/ownership, or surveyor(s) must utilize the Change of Information FIRMS Form, linked to the drop down menu under FIRM. In order to report the closure of a firm or branch office, the Firm Closure Form must be utilized (linked to the drop down menu under FIRM).
